Indications and results of cardiac surgery: 5 years of practical experience and prospects in Mali

Abstract:

Introduction: The André FESTOC Center is a new cardiac surgery service offered by the French humanitarian association Chain of Hope. 4000 patients are waiting for heart surgery. The Festoc Center is a response to this demand for cardiac surgical care in Mali and the sub-region.

Aim: To report the preliminary results of an emerging cardiac surgery center in a country with limited resources.

Materials and Methods: This was a mono-centric, descriptive retrospective study taking place at the Andre Festoc Center from September 10, 2018 to September 30, 2023 (60 months). All patients operated on for congenital (1st group) and acquired (2nd group) heart disease with or without extracorporeal circulation (ECG) were included. Epidemiological, clinical, paraclinical, operative and postoperative data were analyzed using Epi Info7, SPSS, Stata and Excel.

Results: A total of 691 patients underwent surgery, 462 with congenital heart disease and 228 with acquired heart disease. Cannulation was aorto-bicaval and cardioplegia was crystalloid in 60% of cases and blood in 40%. In congenital heart disease, the following were noted: CIA (12.69%) PCA (21.63%) CIV (15.83%) Tetralogy of Fallot (11.59%) Single ventricles (1.72%) CAV (1.25%) CIV+ Pulmonary stenosis (2.19%) Sub-aortic membranes (2.82%), Other (3.13%). The procedures performed were CIA closure, CIV closure, PCA closure, CAV closure, complete FALLOT tetralogy cure, partial cavo-pulmonary bypass. For acquired heart disease, procedures were dominated by mitral valve replacement or plastic surgery, or aortic valve replacement; associated or not with tricuspid plastic surgery: RVM+PT (43.66%), RVM (26, 76%), RVA (8, 45%); PM (11 ,73 %).

Conclusion: In Africa, rheumatic disease is still a topical issue and constitutes a public health problem due to the frequency and severity of its cardiac complications. Cardiac surgery is booming in developing countries.
