Title : Chemical detergent anionic surfactant-inducing dry cardiomyopathy (Yasser's cardiomyopathy) with parallel pupils/CXR phenomenon (Yasser's ophthalmic phenomenon), up-down Yasser's alternance, S1Q3 flat T3 pattern - Dramatic response to honey with possible cardiac regeneration - A case report
Abstract:
Background: Drug- and toxin-induced acute emergency diseases are a common problem in clinical medicine. A concern for dysfunction due to inhalation of chemical detergent anionic surfactant not a commonly reported toxicity. Cardiomyopathy has subcategories that may have a serious risk in cardiovascular medicine. The relationship between the eye and heart is more complex. Electrical alternans is an ECG phenomenon of alternation of QRS complex amplitude or axis among QRS complexes and a wandering baseline, which can be caused by cardiac tamponade and severe pericardial effusion. S1Q3T3 pattern in ECG is a known, classic, non-specific sign of acute pulmonary embolism. Honey bees have an essential role in tissue regeneration.
Case presentation: An 11-year-old Egyptian female student was presented to the intensive care unit (ICU) for toxic cardiomyopathy and tachycardia. This occurred accidentally after inhalation of a chemical detergent, an anionic surfactant (Oxy oil), during cleaning of the home flooring. The girl underwent follow-up for 3 years with periodic serial clinical examination, bilateral eye images, ECGs, plain PA CXR films, and echocardiography were done. A dramatic response occurred after drinking honey post failure of traditional management.
Conclusion: Anionic surfactant induced dry cardiomyopathy makes this surfactant a serious toxin. Yasser’s cardiomyopathy, Parallel Pupils/CXR phenomenon (Yasser’s ophthalmic phenomenon), Up-down Yasser’s alternance, S1Q3 Flat T3 pattern (Yasser's pattern), and RBBB ectopic beats (Yasser’s ectopic), and their reversal are newly described medical innovations. A dramatic response of dry cardiomyopathy to drinking honey bees’ post failure of traditional management would be encouraging its use in like cases. Cardiac regeneration is a suggestive interpretative theory.
