Abstract:

Cardiovascular disease (CVD) remains the leading cause of mortality worldwide despite significant advances in pharmacological therapies and preventive cardiology. Traditional dietary recommendations have largely relied on population-based guidelines, assuming that individuals respond similarly to nutritional interventions. However, growing evidence demonstrates substantial interindividual variability in metabolic responses to identical dietary patterns due to differences in genetics, epigenetics, gut microbiome composition, metabolic phenotype, lifestyle behaviors, age, sex, and environmental exposures. Precision nutrition has emerged as an innovative, evidence-based approach that seeks to tailor dietary interventions according to an individual's unique biological, clinical, and behavioral characteristics. By integrating multi-omics technologies with digital health and continuous metabolic monitoring, precision nutrition offers the potential to optimize cardiovascular risk reduction beyond conventional dietary counseling. Precision nutrition improves cardiometabolic health through individualized dietary strategies, biomarker-guided interventions, and emerging digital technologies.
Current literature evaluating precision nutrition includes studies investigating nutrigenomics, nutrigenetics, metabolomics, lipidomics, gut microbiome profiling, wearable health technologies, and artificial intelligence-assisted dietary recommendations. Particular emphasis was placed on research evaluating cardiovascular risk factors including hypertension, dyslipidemia, obesity, insulin resistance, type 2 diabetes, chronic inflammation, endothelial dysfunction, and atherosclerosis. Personalized dietary approaches including Mediterranean, DASH, plant-predominant, and low-glycemic dietary patterns were evaluated in relation to individual metabolic phenotypes and biomarker responses.
Precision nutrition represents a paradigm shift from generalized dietary recommendations toward individualized nutrition therapy. Emerging evidence suggests that personalized nutrition can improve blood pressure regulation, glycemic control, lipid metabolism, systemic inflammation, endothelial function, body composition, and overall cardiovascular risk by matching dietary interventions to individual metabolic characteristics. Integration of genomic information with circulating biomarkers, gut microbiome analysis, and continuous glucose monitoring enables clinicians to identify patients who may benefit from targeted nutritional modifications. Artificial intelligence and machine learning algorithms further enhance personalization by predicting individual glycemic and lipid responses to specific foods, allowing more accurate dietary prescriptions. Additionally, biomarkers such as apolipoprotein B, lipoprotein(a), HDL functionality, inflammatory markers, and metabolomic signatures may improve cardiovascular risk stratification and therapeutic monitoring. Lifestyle behaviors—including physical activity, sleep quality, stress management, and smoking cessation—remain essential complementary components of precision nutrition and significantly influence long-term cardiovascular outcomes.
Precision nutrition represents one of the most promising advances in preventive cardiology by recognizing that optimal nutrition is highly individualized rather than universally prescriptive. As genomic sequencing, metabolomics, microbiome science, wearable technologies, and artificial intelligence become increasingly integrated into routine clinical practice, personalized nutrition has the potential to transform cardiovascular disease prevention and management. Future large-scale randomized clinical trials are needed to establish standardized precision nutrition algorithms, validate clinically actionable biomarkers, and evaluate long-term cardiovascular outcomes. Successful implementation will require multidisciplinary collaboration among cardiologists, obesity medicine specialists, lifestyle medicine physicians, dietitians, exercise professionals, behavioral psychologists, and data scientists. Ultimately, precision nutrition may serve as a cornerstone of personalized cardiovascular medicine, enabling clinicians to deliver targeted, evidence-based interventions that improve metabolic health, reduce cardiovascular events, and advance the delivery of truly individualized healthcare.
